>This is the rebate form.  (Its the very last page of the PDF file).  Remember last month I had a review and then a giveaway of the National Geographic Big Book of Fun?  It’s really a nice activity book!  You do need to have 4 UPC’s from certain products in order to get the rebate but they are all easy to find products!
