This is the link to the rebate form.  Be sure to make a copy before sending in.  You just never know when something goes awry when you mail a rebate in- mine got mixed up 2 years ago and it was a $75 gift card they said I hadn’t fulfilled my rebate requirements- I had, but somehow one receipt was missing in the envelope.  Had I not copied it, I’d have been out the $75 rebate!!
